I am an in-network provider for Cigna, United, and Sutter Select health insurance plans. I may also be able to bill for some other PPO plans as an out-of-network provider. Click here to check your benefits.
CANCELLATION POLICY
I request 48 hours notice if you cannot make it to your appointment. Any appointments cancelled with less than 48 hours notice will be subject to a cancellation fee. This cost is not reimbursable by insurance.
